Welcome to Beacon Pediatrics! We are excited to welcome your family to our practice. This page provides information for families who are interested in becoming a new patient or who need to request or transfer medical records.
Our New Patient Process
We look forward to getting to know you and your child! Before your first visit, please complete the required new patient forms and provide the requested insurance and medical information.
1. Complete the New Patient Packet
The first step is to complete our New Patient Packet with all requested information about your child, including demographics, insurance, medical history, medications, allergies, and other important health information. Click the button to the right to get the New patient Packet.
Please print or download an additional page 2 of the packet for each child you are looking to establish.

2. We Create Your Child’s Chart
Once we receive the completed new patient information, our team will create your child’s chart in our system.

3. Request Previous Medical Records
We can provide a medical records release form for you to complete or you can print it off and turn in with the New Patient Packet. This authorizes your child’s previous pediatrician or healthcare provider to send their medical records to Beacon Pediatrics. Click the button to the right to get the Medical Record Release Form.

4. Records Are Received & Reviewed
Once we receive the records from your child’s previous pediatrician, they are reviewed by our medical records member. This allows us to better understand your child’s medical history and determine that we have the appropriate information needed to establish care. We prefer to have full medical history.

5. Schedule Your First Appointment
After the records have been received and reviewed, a member of our Front Desk team will contact you to schedule your child’s first appointment.

Submitting a New Patient Packet does not mean an appointment has been scheduled. An appointment will be scheduled after the required medical records have been received and reviewed.

The time required to complete this process may vary depending on how quickly your child’s previous pediatrician responds to the medical records request.

Please do not call to schedule an appointment before the records have been received and reviewed. Our Front Desk team will contact you once your child is ready to be scheduled.
We appreciate your patience while we work to make sure we have the information needed to provide your child with safe and informed care.
Medical Records
Requesting Records from Beacon Pediatrics
If you need a copy of your child’s medical records, please complete a medical records release form with the reason for request.

Medical records may be released to a parent, legal guardian, healthcare provider, school, or other authorized individual or organization in accordance with applicable privacy requirements.
Transferring Records to Another Pediatrician
If your child is transferring care to another pediatrician, you will need to complete a medical records release form with all of the information of you new office on said form. This allows Beacon Pediatrics to send the requested records to the new healthcare provider.

Please allow adequate processing time for medical record requests. Medical record requests may take up to 45 days to process, depending on the type of request and circumstances involved.
Sports, School, Daycare & Camp Records
If you need immunization records, physical forms, health appraisals, or other medical documentation for school, daycare, camp, or another organization, please contact our office.
Privacy & Authorization
Medical records are protected health information (PHI). We will verify the identity and authorization of the person requesting or receiving records before releasing information.
